The following is a checklist of required items, forms and standard details for exterior windows and doors replacement only. Please note that this checklist is not intended to be all-inclusive. Due to changes in code, regulations and ordinances, other requirements may apply.

  “This permit for windows and doors, is limited solely to authorizing that work. Further, this permit may not be used as record evidence that any portion of the structure to which the windows and doors are attached was lawfully permitted notwithstanding this permit’s inclusion in the City’s records. • Provide elevations of each exterior wall having windows and doors being replaced. 
• Provide a floor plan and identify which windows and doors are being replaced. A
• Current Miami Dade County  Notice of Approval (NOA) Product Approval or State of Florida Department of Business and Professional Regulation (DBPR) Product Approval.

Information needed for the window and door replacement:

• Code Sections in Bold.
• Room label and location
• Windows and Doors schedule, identify which window or door relate to which NOA.
• Size and model of each window and door. (Shall be existing frame or existing rough opening, changes to the openings are not allowed for Homeowner’s Express Reviews, they will need to be part of a remodeling permit).
• Identify emergency escape and rescue for sleeping rooms, FBC R310.1.
• Identify windowsill height to finish floor and windowsill height to exterior ground, for each window being changed. FBC R312.2.1.
• Site wind pressures. ASCE 7-22 Chapter 30 and the 2023 FBC, Building sections 1620.2 and 1620.3 (175 mph (3-second gust)/ Exposure C). Use this link for BORA Interpretation Wind Load Table for Openings.

Make sure to identify the following items in NOA:

• Required positive and negative pressures of each window/door being used (typically located in the product approval documents under the design and pressures rating (psf).
• Identify the size of each window or door.
• Identify all mullions.
• Identify which glass type and thickness.
• Cannot be expired, must be for the current Florida Building Code in effect. Use this link for Product Control Search.

This permit is for repairs, reconstruction or renewal of any part of an existing building for the purpose of its maintenance or to correct damage, not for alterations. Example: Floor plans with electrical connections, appliance location, dimensions. Elevations with counter heights and outlet locations. 

This type of permit applies for interior repairs consisting of: 

• Replacement of existing cabinetry 
• Replacement of appliances  
• Repairs  
• Electrical outlets  
• Plumbing connections. No new locations. 
• Repair of existing bathrooms  
• Replacement of plumbing fixtures

Applicant Checklist: Standalone Driveway Permit

Required Sheets:

• Survey (less than 1 year old)
• Site plan

Both survey and site plan need to show information (which must match between survey and site plan):

• Legal description
• Boundary dimensions
• Base Building Line (BBL)
• Distance to Center Line
• All right of way conditions (e.g. trees, poles, guy wires, hydrants, curb, gutter, sidewalk, meter boxes, fences)

In addition, site plan must show:

• Proposed driveway location
• Distance from edge of driveway to side property line
• Driveway width and length
• Driveway flares with dimensions
• Visibility triangle (10’ line along BBL, 10’ line along driveway edge into private property, and a line connecting them)
• Copies of relevant Public Works standard details

Note: A copy of a boundary survey marked up with proposed driveway may be acceptable as a site plan. If so, it must be submitted in addition to a survey without markups.

Required Inspections:

• Public Works final inspection
• Formwork inspection is optional

Additional Permits Needed (to be pulled by contractor):

• Public Works sub-permit
• ROW closure permit

Not eligible for express plan check if:                                                                                

• More than 2 dwelling units (bigger than duplex)
• After the fact/legalization permit
• Permit triggers tree/utility pole relocations (new driveway at location of existing feature)

Nonstandard Materials: Pavers, concrete slabs with spaces in between, and stamped concrete all require covenants to make property owner responsible for maintenance instead of the City.

Criteria for Approval:

• Edge of driveway at least 5’ from side property line
• Driveway width between 10’ and 40’
• Slope requirements: max 2% cross slope and 5% longitudinal slope for sidewalk, max 8.3% slope for vehicular ramp

Below is a list of information that should be included in the plans. The specifics may vary depending on the case: 

• Provide Plans: floor plan, elevations, sections, any details needed for the interior demolition and any other drawings necessary. 
• Provide Scope of Work: Clearly define which areas of the house will be demolished (e.g., walls, floors, ceilings). 
• Provide Existing Layout: A detailed drawing showing the current state of the interior. 
• Proposed Layout: Indicates how the space will look after demolition. 
• Provide Structural Plans: For modifications to load-bearing walls or other structural components, plans must be reviewed and sealed by a licensed professional. 
• Provide Temporary Support Details: If applicable, include how structural integrity will be maintained during demolition. 
• Mechanical, Electrical and Plumbing Plans: Utility Disconnection/Capping. Show where utilities like plumbing, gas, and electrical systems will be disconnected or safely terminated.

Applicant Checklist: Tree Permit

 Factors to evaluate when determining whether a Tree Permit is required:

• The Tree Boundary Survey shows trees on site within close proximity to the proposed scope of work.
  • Close Proximity: The proposed work is located within 20 feet of existing trees.
• The Site Plan and/or Tree Disposition Plan lists existing trees to remain, remove or relocate.
• Aerial imagery (Google, Property Appraisal, etc.) shows existing trees within close proximity of the proposed work.
• There are shared/neighboring trees on adjacent lots that could be impacted by the proposed scope of work. (Proposed development can potentially affect trees on adjacent lots).

Applicant Checklist: Tree Permit

Required Sheets:

• Tree Boundary Survey (no older than 12 months at the time of submittal).
• Tree Disposition Plan: identifies and lists trees within close proximity of the proposed scope of work.
• Tree Protection Plan: Identifies all trees to remain on site with adequate. Tree protection and protection barriers in accordance with the Miami-Dade County Landscape Manual
• Tree Report, when applicable: Provide an assessment of trees, including tree details. The assessment must be done by an ISA Certified Arborist.
