Miami continues to face the highest HIV infection rate in the United States. In 2020 alone, more than 25,000 people in Miami-Dade County were living with HIV. The City of Miami is taking action — committed to reducing, and ultimately eliminating, the spread of HIV while providing vital support for those affected.

Through free testing, education, outreach, and access to care, our HIV Initiative connects residents with the tools they need to stay informed, protected, and healthy.

HIV (human immunodeficiency virus) is a virus that weakens the immune system by attacking the body’s natural defenses. If untreated, HIV can progress to AIDS (acquired immunodeficiency syndrome). While there is currently no cure, early diagnosis and consistent treatment can keep the virus under control and allow individuals to live long, healthy lives.

Thanks to advancements in medicine, people with HIV who receive proper care and maintain an undetectable viral load for at least six months cannot transmit the virus to their sexual partners. This breakthrough is known as U=U: Undetectable = Untransmittable—a powerful message of hope, health, and prevention.

Know the Signs.

In the first few weeks after infection, some individuals may experience flu-like symptoms—fever, fatigue, sore throat, or rash. These symptoms may last a few days to weeks, or may not appear at all. That’s why routine testing is essential.

The only way to know your status is to get tested. Knowing your status empowers you to protect yourself and others, access timely care, and take charge of your health.

HIV in Miami-Dade County

Prevalence and New Diagnoses

  • As of 2022, 28,749 individuals were living with an HIV diagnosis in Miami-Dade County, marking it as a significant public health concern.
  • In 2020, there were 25,595 people living with HIV in Miami-Dade County.
  • The HIV incidence rate in Miami-Dade County was 28.4 per 100,000 population, higher than the state average.

Demographic Disparities

  • In 2017, 74.3% of diagnosed HIV cases were among men, with 88% of all cases among Hispanic and non-Hispanic Black residents.
  • The rate of new HIV diagnoses in 2019 was:
    • 69.6 per 100,000 among non-Hispanic Black individuals
    • 38.5 per 100,000 among Hispanic/Latinx individuals
    • 26 per 100,000 among non-Hispanic White individuals

Care Continuum (2021)

  • 67% of people living with HIV (PLHIV) were on antiretroviral therapy (ART).
  • Among those on ART, 94% achieved viral suppression.

Mortality

  • In 2020, there were 129 deaths related to HIV in Miami-Dade County.

The City of Miami is proud to be part of Fast-Track Cities, a global initiative committed to ending the HIV epidemic by 2030. This partnership includes over 300 cities worldwide, united in advancing data-driven, equity-focused strategies to reduce HIV infections, improve health outcomes, and eliminate stigma and discrimination.

Miami has joined local leaders, health departments, service providers, and community advocates to accelerate progress toward critical HIV targets. The initiative originally focused on the 90-90-90 goals—90% of people with HIV diagnosed, 90% of those on treatment, and 90% of those achieving viral suppression—but now moves beyond, striving for zero new infections and zero AIDS-related deaths.

Through the support of global partners—including IAPAC, UNAIDS, UN-Habitat, and the City of Paris—Miami benefits from:

  • Technical assistance and capacity-building for local providers and organizations
  • Transparent data reporting through the Fast-Track Cities Global Web Portal
  • Efforts to eliminate HIV-related stigma in healthcare settings
  • Quality-of-life assessments for individuals living with HIV
